Custom Layer from Image Server

I'm trying to add two custom layers from Arcgis image servers. I found the following discussion and was wondering about the outcome. (1) the thread mentions potential licensing issues; was it determined that those issues were founded? and (2) did the modified link at the bottom work (ie workaround the auto-configure limitation)?

I'd like to create custom layers from these sources, but need a little help in getting them to parse without auto-configure. Could you help me understand how to do so?

    On licensing, per our terms of service ( by bringing user content onto our platform you represent that you have the right to do so and appropriate licensing, etc.

    For the URL, ImageServer's cannot be autoconfigured by CalTopo. You'll need to have a template URL for this server. Both of those servers seem to beyond a login wall. You'll need a templated URL typically they look like one of these:{Z}/{X}/{Y}.png{left},{bottom},{right},{top}&WIDTH...
